Congratulations on the planned baptism of your child! Holy Name Cathedral is pleased to be the Church in which this first Sacrament will be celebrated. After reading the details below, please complete the Baptism Registration Form.
*If you are a registered parishioner (you regularly get mail from us), go ahead and fill out the baptismal form. If you are not a registered parishioner, please call or email Dawn Swanson at 312-573-4401 or email@example.com before filling out the form. In special cases (grandparents are registered parishioners, previous children were baptized at Holy Name, parents were married at Holy Name) we will consider scheduling the baptism here. However families who live outside of Holy Name parish should consider having their baby baptized at the neighborhood Church.
Baptisms will be held in the Cathedral at 2:00pm on the following Sunday in 2014 with the expection of April 13 (2:30pm - Palm Sunday)

If this is your first child, we require that you take a Preparation Class prior to the Baptism. Classes can be taken before or after the birth of your child. Babies are welcome at the class. The course is strongly recommended for any godparent who has not previously taken it.
*Please note: You MUST register in advance in order to attend a baptismal class due to limited space. THAT MEANS NO WALK-INS.
*We offer 3 options for baptismal preparation classes.
Friday classes will be held from 7:30-8:30pm
We have added two classes on Saturday for your convenience 1:00-2:00pm or 2:30-3:30pm (you only need to attend one class).
*When filling out the baptismal registration form, please put the time you prefer if you are attending a Saturday Baptismal Preparation Class.

The Church requirements for Roman Catholic Godparents are:
A child must have one practicing Roman Catholic godparent in good standing with the Church. This Catholic godparent usually must be at least 16 years old and be mature enough to understand, accept and carry out the duties of a godparent.
- A godparent must have received the Sacraments of Initiation: Baptism, Eucharist and Confirmation.
- A godparent must be living an exemplary life consistent with faith and morals as taught by the Roman Catholic Church.
- A godparent must be actively practicing the faith.
- A non-Catholic, baptized Christian may be chosen as a Christian Witness, as long as one godparent is Catholic.
Each family is allowed to have as many guests as they would like. You will be asked to give the number of guests you expect so that we can reserve that number of seats together.
It is appropriate to make a donation to Holy Name Cathedral at the time of the Baptism. There is no set amount.
Parking is provided during the ceremony at the Cathedral Parking lot on the corner of State and Superior. The entrance to the lot is on Superior.
